Your Opportunity:

Do you thrive on bringing groups of people together to solve complex problems? Are you ready to combine your skills in quality improvement, implementation, and change management to support system level change? Join a passionate and diverse team of system transformers that work towards integrating primary health care within Alberta’s refocused health system. We work to support others to design and implement new or improved ways of delivering health care that work for people in the community. We coordinate the implementation of provincial primary health care integration priorities by bringing expertise in access and quality improvement, behavior change management and patient centered practices. We meaningfully engage patients and families to help shape and transform the health care system.

Description:

Reporting to the Program Lead, System Integration and Optimization within the Clinical Services Integration and Patient Flow Supports (CSIPFS) department, the Senior Consultant leads system integration, improvement, and implementation of complex provincial initiatives. This position requires an experienced Senior Consultant with strong working knowledge of primary care and who can work with a high degree of independence within a virtual team environment. Recognized as a high-impact agent of change, this senior consultant supports strategic priorities by applying expertise in change management, facilitation, co-design, and data-informed decision-making to improve patient and provider experiences and enhance system performance. This role works independently and collaboratively with internal and external partners (including patients, providers, and system stakeholders) to design, implement, and scale integrated healthcare solutions. The work and projects assigned to this position will primarily be focused within Transitions in Care and will require the incumbent to have a foundational level of knowledge for the core components and operational practices included in the Home to Hospital to Home (H2H2H) Transition in Care Guideline. A secondary focus area for this position will require experience in providing secretariat support to provincial level cross agency committees. Other work assignments are assigned across the team and broader department as capacity and demand for senior consultant support and skills are identified. This position requires availability to travel to Calgary hospital sites to provide ad hoc onsite support to a small team of PCA Integrated Care Leads.
