Description:

Reporting to the Program Lead, System Integration and Optimization within the provincial Clinical Services Integration and Patient Flow Supports (CSIPFS) department, the coordinator provides coordination and operational support to initiatives focused on service integration, patient flow optimization, and system improvement across the care continuum. The work and projects assigned to this position will primarily be focused within Transitions in Care and will require the incumbent to have a foundational level of knowledge for the core components and operational practices included in the Home to Hospital to Home (H2H2H) Transition in Care Guideline. A secondary focus area for this position will require familiarity of the Health Change Alberta model and program. Other work assignments are assigned across the team and broader department as capacity and demand for coordinator support and skills are identified. Current priority focusses areas the coordinator will contribute to includes: Refresh of the H2H2H Guideline content, Improving follows up to primary care for higher risk patients after a hospital stay, Operations support and planning activities for the Integrated Care Program Health Change Alberta online learning environment management and optimization The coordinator works within collaborative team environments and provides their skills toward organizing, documenting, communicating, and tracking program activities that enable the project teams. This position works closely with internal PCA teams, and external partners to support coordinated implementation while maintaining alignment with PCA, portfolio, and department priorities and system objectives.

Required Qualifications:

Post-secondary education in a health discipline, education, social sciences, project management or related field and/or equivalent experience. Minimum of 5 years' experience working within complex health care environments. Experience working within roles that demonstrate maintaining internal and external partnerships and collaborations. Ability to confidently and effectively collaborate and consult with a variety of partners. Comfortable developing relationships remotely. Strong written and verbal communication skills. Strong critical thinking and problem-solving skills. Ability to work productively on multiple projects simultaneously while meeting deadlines Formal training and certification in change management, project management and/or improvement methodologies. Strong competencies using Microsoft suite of tools (Outlook, PowerPoint, Word, Excel, Access, SharePoint, CoPilot, etc.).

Preferred Qualifications:

Knowledge and experience with software to support project management, data management and data analysis preferred. Formal training or certification with demonstrated experience in change management, project management, and quality improvement methodologies. Advanced competencies using Microsoft suite of tools (Outlook, PowerPoint, Word, Excel, Access, SharePoint, CoPilot, etc.).
