As a Social Worker within our Public Health, Primary Care, and Chronic Disease portfolio, you will be part of an integrated team that recognizes health is shaped by complex social realities and will be able to help patients and clients navigate systems, reducing barriers to care, and ensuring prevention, early intervention, and support are grounded in equity and compassion. In this role you will work within a variety of care settings that span the portfolio and will be supporting individuals and families facing challenges such as housing instability, financial strain, and family stressors – factors that may directly influence their ability to engage in care. Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
