Social Worker II
Your Opportunity:
You’ll be stepping into a role that is at the heart of Primary Care Alberta’s mission to help Albertans live their healthiest lives and help residents of Alberta navigate their health journey. As a Social Worker within our Public Health, Primary Care, and Chronic Disease portfolio, you will be part of an integrated team that recognizes health is shaped by complex social realities and will be able to help patients and clients navigate systems, reducing barriers to care, and ensuring prevention, early intervention, and support are grounded in equity and compassion. In this role you will work within a variety of care settings that span the portfolio and will be supporting individuals and families facing challenges such as housing instability, financial strain, and family stressors – factors that may directly influence their ability to engage in care. This opportunity is your chance to shape healthier pathways for Albertans by ensuring that the social determinants of health are embedded in how we deliver care.
