Description:
Reporting to the Program Lead, System Integration and Optimization within the Clinical Services Integration and Patient Flow Supports (CSIPFS) department, the Senior Consultant leads system integration, improvement, and implementation of complex provincial initiatives. This position requires an experienced Senior Consultant with strong working knowledge of primary care and who can work with a high degree of independence within a virtual team environment. Recognized as a high-impact agent of change, this senior consultant applies an unique skillset in adult learning and educational design to build, test, evaluate, and adapt solutions to meet the learners’ needs. Familiar with new and innovative knowledge translation models and educational technologies, they are skilled at applying principles in adult learning, human factors design, and systems thinking to create meaningful learning experiences across a variety of virtual learning platforms. This role works independently and collaboratively with internal and external partners (including patients, providers, and system stakeholders) to design, implement, and scale integrated healthcare solutions. The work and projects assigned to this will primarily be focused within Transitions in Care and will require the incumbent to have a foundational level of knowledge for the core components and operational practices included in the Home to Hospital to Home (H2H2H) Transition in Care Guideline. Current projects include leading the enhancement of the H2H2H Guideline implementation guide and supporting the Guideline Refresh project including writing, engagement, validation of content, and final publication. Other work assignments are assigned across the team and broader department as capacity and demand for senior consultant support and skills are identified.

Baccalaureate level degree with minimum of 5 years of experience within a healthcare related field. Formal training and demonstrated experience related to adult learning and/or educational design. Formal training and demonstrated experience in change management, project management, and quality improvement methodologies.

Strong change management and quality improvement skills. Strong writing skills with ability to adapt to appropriate level and audience. Ability to work independently and collaboratively within a team. Relationship-building and conflict resolution skills. Proficiency in Microsoft Office 365 tools including Co-Pilot.
Preferred Qualifications:Master level degree in communications, education or health-related discipline. Equivalencies in training and experience may be considered. Knowledge and experience with software to support project management, data management and data analysis. Knowledge and experience with design programs or applications and video production skills. Advanced competencies using Microsoft Office 365 tools including Co-Pilot.
