Registered Nurse - Chronic Disease Management
Your Opportunity:
The Chronic Disease Management (CDM) Nurse utilizes the nursing process, critical thinking and problem solving skills to provide care to clients across the life span in the area of chronic disease management. The CDM Nurse ensures the development, implementation and ongoing administration of the client care plans. The CDM Nurse assesses, coordinates, supervises, administers and evaluates the provision of health services and healthcare resources. The CDM Nurse accepts responsibility for continuing competencies through ongoing professional development, including participation in education programs, research and continuous quality activities. The CDM Nurse demonstrates the ability to accurately and effectively assess client status and to take appropriate intervention measures that have significant impact on the client's medical condition, safety, as well as ultimate recovery. The CDM Nurse may provide education and support in the areas of diabetes, weight management, cardiac risk reduction, secondary stroke prevention, and lifestyle management. In addition to a competitive rate of pay, Primary Care Alberta currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of PCA. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:Formal Education: RN required. Knowledge, Skills, Abilities and Competencies: Nursing standards/protocols as required in area of nursing specialty (diabetes and/or chronic disease management) related to the job assignment. Balance numerous demands and conflicting priorities. Work within a team environment and to effectively contribute to client care planning processes and improvement in service delivery. Communication skills and ability to adapt communication style to meet the needs of specific clients and to relay client information to physicians, who may be geographically removed, in a concise, accurate manner in order to facilitate the physician's treatment decisions. Assessment skills to recognize and respond appropriately to client change in status. Strong critical thinking skills; ability to prioritize in fast paced clinical setting.
Preferred Qualifications:Baccalaureate degree in nursing from an accredited academic institution preferred; graduate from an accredited Registered Nursing program with related experience will be considered.
