Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)

Additional Required Qualifications:

Minimum of 5 years of recent clinical experience required. Minimum of 3 years’ experience in advanced discharge planning &/or care planning, developing & implementing innovative solutions. Understands & applies the principles of case management. Experience &/or knowledge of community resources including working knowledge of Calgary Zone PCNs. Experience working with individuals with mental health issues, addictions, unhoused, &/or challenges navigating the health care system. Experience & training in Trauma Informed Care. Understands the impact of social determinants of health on patients’ lives. Intermediate to advanced computer skills with keyboarding speed of 45 wpm or greater. Proficient in MS Suite; able to navigate REDCap with training as well as EMRs such as EPIC. Exceptional interpersonal & communication skills. Ability to build rapport quickly with patients/clients & other health care team members. Ability to work effectively with multiple teams but also independently. Ability to de-escalate patients with aggressive predispositions. Clear understanding of boundary setting in patient care. Ability to prioritize tasks & manage time effectively. Demonstration of adaptability to change. Meet the physical requirements of the position: walking long distances while pulling or carrying bag, ability to take stairs, ability to lift or carry up to 25 lbs, ability to push or pull up to 40 lbs, ability to maintain standing/sitting for 1 hour at a time.

Preferred Qualifications:

Recent experience, within last 2 years in chronic disease management. Experience in both acute care and community-based programs.
