Health Link is seeking an experienced LPN to support the Specialized Outpatient Geriatric Medicine Program. Reporting to the Manager, Health Link, the Licensed Practical Nurse plays an integral role in providing safe, effective, and patient- and family-centred care to older adults. The LPN receives and reviews referrals from healthcare providers, assesses referral information, supports care planning, and helps connect patients and families with appropriate healthcare services and community resources. Working primarily within Connect Care, the LPN applies specialized nursing knowledge, clinical judgment, critical thinking, and problem-solving skills to determine patient needs and facilitate access to geriatric services. The role includes collaborating with interdisciplinary team members and liaising with booking and scheduling teams to coordinate timely patient appointments and ensure a seamless care experience. The successful candidate will demonstrate excellent communication skills, strong organizational abilities, and a commitment to quality patient care. This position requires the application of nursing principles related to informatics, ethics, legal and professional standards, primary healthcare, Patient and Family-Centred Care, and PCA values. As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and PCA policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of PCA.

Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:Minimum of three (3) years of clinical nursing experience, within the last four (4) years. Minimum of two (2) years of recent experience providing patient education and health information to diverse populations, within the last three (3) years. Demonstrated proficiency with computer applications, electronic clinical documentation systems, and web-based programs. Minimum typing speed of 35 words per minute and intermediate computer skills. Strong verbal, written, and active listening skills with the ability to adapt communication styles to meet patient and family needs. Ability to communicate patient information accurately, concisely, and professionally to healthcare providers and interdisciplinary team members. Proven ability to build professional, safe, respectful, and inclusive relationships. Effective use of information and communication technology to support patient- and family-centred care. Strong negotiation, conflict resolution, and complex critical-thinking skills in a dynamic and changing environment.
Preferred Qualifications:Experience providing patient teaching and health information by telephone or through virtual platforms. Experience working with older adults and/or within geriatric medicine programs. Experience using Connect Care or other electronic health record systems. This position is ideal for an LPN who enjoys coordinating care, working collaboratively with interdisciplinary teams, and making a meaningful difference in the health and well-being of older adults.
