Community Health Representative Worker

Rural Public Health is seeking a compassionate and community-minded Community Health Representative to join our team in La Crete, Alberta. In this unique entry-level role, you will work closely with members of the Low German-speaking Mennonite community, serving as a liaison between individuals, families, health care providers, and community organizations. Your fluency in Low German (Plautdietsch) and strong interpersonal skills will help bridge communication gaps, foster understanding, and support positive health outcomes. As a Community Health Representative, you will empower clients to take an active role in their health and well-being by promoting healthy living, disease and injury prevention, immunization awareness, chronic disease support, parenting and prenatal programs, and other community health initiatives. You will collaborate with members of the larger Public Health team, Primary Care Alberta programs, physicians, educators, municipalities, Family and Community Support Services, and other local partners to ensure community members have access to culturally appropriate services and support. You will also help identify emerging community health needs, advocate for clients and families, facilitate communication between health care providers and clients, and contribute to the development of responsive and culturally relevant health services. This position requires travel throughout the LaCrete area, meaning a valid Alberta Driver's License and reliable vehicle are required. The regular work schedule is Monday to Friday, 8:30 a.m. to 4:45 p.m., with occasional evenings or weekends to support community events and outreach activities. Completion of Grade 12.
Additional Required Qualifications:Experience working with members of the Low German Mennonite community. Experience communicating fluently in Low German (Plautdietsch). Northern Allowance: In addition to a competitive rate of pay, PCA currently has a Northern Allowance in place to an annual maximum of $6,300. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay. Travel Expense Reimbursement: Permanent and temporary employees working in excess of one (1) year whose work sites are above the 57th parallel are also eligible to receive an annual reimbursement of personal travel expenses to an annual maximum of $1,235. Eligible employees will be reimbursed for the cost of one (1) round trip, per calendar year, to any destination within Alberta. Travel must originate from a community above the 57th parallel to any destination within Alberta and cannot be carried over.
Preferred Qualifications:Experience working in partnership with community groups and agencies. Experience in human services field. Previous experience working in health care (eg Health Care Aid).
